Position Summary

Position Summary
- Builds and leads A Place Meant's supported and customized employment program under our Nebraska VR service agreement. Carries a caseload of job seekers, develops employer partners, and provides job coaching. Also completes monthly Shared Living home visits and referral follow-up. Reports to the Operations and Compliance Lead.
Essential Functions
- Complete Discovery with each job seeker to learn their strengths, interests, and support needs.
- Write the Plan for Job Development and carry out job search and placement activities.
- Develop relationships with local employers, and create or negotiate jobs that match each person's skills.
- Provide on-site job coaching and support, and fade support as the person becomes independent.
- Teach job readiness skills in our computer lab, including resumes, online applications, interview practice, and digital skills.
- Communicate with VR specialists, service coordinators, families, and employers. Attend team meetings as needed.
- Only provide services that VR has authorized in advance.
- Complete a monthly visit to each Shared Living home and follow up on new referrals for Community Integration, Supported Family Living, and employment services.
- Take part in community outreach and the after-hours on-call rotation.

- Complete all Nebraska VR reports, activity notes, and verification of hours accurately and on time so services can be billed.
- Use Therap and other approved systems. Follow Nebraska VR, Nebraska DD, and APM policies.

Minimum Qualifications
- At least 1 year of experience in job development, job coaching, supported employment, or case management with people with disabilities.
- CESP (Certified Employment Support Professional) or ACRE certificate preferred, or willing to earn one.
- Associate or bachelor's degree in human services, rehabilitation, social work, psychology, or a related field preferred. Equal experience is welcome.
- Confident reaching out to employers. Organized, dependable, and good with deadlines and reports. Comfortable with computers.
- Valid driver's license, reliable vehicle, and car insurance. At least 19 years old.
